Queen Elizabeth II Diamond Jubilee Medal

A new commemorative medal was created to mark the 2012 celebrations of the 60th anniversary of Her Majesty Queen Elizabeth II’s accession to the Throne as Queen of Canada. The Queen Elizabeth II Diamond Jubilee Medal is a tangible way for Canada to honour Her Majesty for her service to this country. At the same time, it serves to honour significant contributions and achievements by Canadians.

Eligibility Criteria

To be eligible for this honour, a person must:

• Be a Canadian citizen or a permanent resident of Canada, but need not necessarily reside in Canada;
• Have made a significant contribution to the Deaf community in Canada; and
• Be alive on February 6th, 2012, the 60th anniversary of Her Majesty’s accession to the Throne. The medal can be awarded posthumously, as long as the recipient was alive on that date.

Nomination Process

Canadian Association of the Deaf – Association des Sourds du Canada (CAD-ASC) is honored to present 30 medals to Deaf Canadians and we are soliciting nominations for the Queen Elizabeth II Diamond Jubilee Medal. Provincial Affiliates will nominate two people from its province, CAD-ASC Board of Directors will nominate six Deaf individuals and the general public will be invited to nominate six people.

Nominees should have contributed significantly to Deaf community in Canada. Nominations must provide details and specific instances of nominees’ contributions, as well as supporting documentation.

Nominations must be received by Friday November 2, 2012

How to Submit a Nomination

To nominate someone for Queen Elizabeth II Diamond Jubilee Medal, please fill out the attached form and return it:

1. Submit your form by email or fax. Nomination forms should be submitted as follows:

a. By Email: Send forms to info@cad.ca with the subject header: “Queen Elizabeth II Diamond Jubilee Medal”
b. By Fax: Fax your completed form to 613-565-1207 and address it to “Nominations Committee, Queen Elizabeth II Diamond Jubilee Medal.”

2. We will confirm receipt of your nomination. If we require any further information or your nomination is incomplete, we will follow-up with you.

Once we confirm receipt of your nomination and have ensured that your nomination is complete, you will receive no further communication from us until a decision is made.

Successful Nominees

The names of the winners will be posted on our website and social media after December 15th, 2012.
